Dosing & Reconstitution Guide

Educational guide for reconstitution and weekly dosing

Standard / Gradual Titration (3.0 mL = ~3.33 mg/mL)

Week Weekly Dose Units (per injection) (mL)
Weeks 1–4 2.5 mg (2,500 mcg) 75 units (0.75 mL)
Weeks 5–8+ 5 mg (5,000 mcg) 150 units (1.50 mL)

Frequency: Inject once weekly subcutaneously. This schedule mirrors clinical trial protocols with gradual titration to improve tolerability.[1][2] The 3.0 mL dilution provides practical unit measurements for standard maintenance doses.

Reconstitution Steps

  1. Draw 3.0 mL bacteriostatic water with a sterile syringe.
  2. Inject slowly down the vial wall; avoid foaming.
  3. Gently swirl until dissolved (do not shake vigorously).
  4. Label with date and refrigerate at 2–8 °C (35.6–46.4 °F), protected from light.

Advanced / High‑Dose Protocol (2.0 mL = 5.0 mg/mL)

Week Weekly Dose Units (per injection) (mL) Notes
Weeks 1–4 5 mg (5,000 mcg) 100 units (1.00 mL) 0.5 vials
Weeks 5–8 7.5 mg (7,500 mcg) 150 units (1.50 mL) 0.75 vials
Weeks 9–12+ 10 mg (10,000 mcg) 200 units (2.00 mL) 1 vial

Caution: High‑dose protocols (7.5–10 mg weekly) have been explored in phase 1b research but require careful monitoring and clinical oversight.[1] Doses above 6 mg increase the risk of gastrointestinal side effects. The 2.0 mL reconstitution provides higher concentration to reduce injection volume. For doses requiring >3.0 mL total volume, split into two separate injections at different sites.

Peptide Reconstitution Simulator

Practice preparing this vial with the verified strength and final volume from this protocol.

Mazdutide Vial and Research Context

  • Reconstitute: Add 3.0 mL bacteriostatic water → ~3.33 mg/mL concentration (standard protocol).
  • Typical weekly range: 2.5–5 mg once weekly with gradual titration.
  • Easy measuring: At 3.33 mg/mL, 1 unit = 0.01 mL ≈ 33.3 mcg on a U‑100 insulin syringe.
  • Storage: Lyophilized: freeze at −20 °C (−4 °F); after reconstitution, refrigerate at 2–8 °C (35.6–46.4 °F); avoid freeze–thaw cycles.

Mazdutide, also called IBI362 or LY3305677, is a once-weekly dual GLP-1 and glucagon receptor agonist studied in adults with obesity, overweight, and type 2 diabetes. Randomized phase 1b, phase 2, and phase 3 trials evaluated different populations, dose levels, and durations; those studies do not establish that an independently supplied research vial is interchangeable with a clinical-trial formulation.[1][2][3][4]

Protocol Overview

Concise summary of the once‑weekly regimen.

  • Goal: Support chronic weight management and metabolic improvement through dual GLP‑1/glucagon receptor activation.[2][3]
  • Schedule: Once‑weekly subcutaneous injections for 8–12 weeks minimum (clinical trials ranged from 12–48 weeks).
  • Dose Range: 2.5–5 mg weekly with gradual titration; advanced protocols may reach 10 mg.
  • Reconstitution: 3.0 mL per 10 mg vial (~3.33 mg/mL) for standard dosing; 2.0 mL for higher concentrations if needed.
  • Storage: Lyophilized frozen at −20 °C (−4 °F); reconstituted refrigerated at 2–8 °C (35.6–46.4 °F); follow the exact product’s validated use-by date.

Dosing Protocol

Suggested weekly titration approach.

  • Start: 2.5 mg once weekly for first 4 weeks to establish tolerability.
  • Increase: Advance to 5 mg weekly at week 5; maintain for remainder of protocol (weeks 5–12+).
  • Frequency: Once per week (subcutaneous injection).
  • Cycle Length: Minimum 8 weeks; clinical evidence supports 12–48 week protocols.
  • Timing: Administer on the same day each week; rotate injection sites to avoid irritation.

Storage Instructions

Proper storage preserves peptide stability and potency.

  • Lyophilized: Store at −20 °C (−4 °F) or below in dry, dark conditions; protect from moisture.
  • Reconstituted: Refrigerate at 2–8 °C (35.6–46.4 °F); bacteriostatic water does not establish peptide stability or a universal use-by period.
  • Important: Do not freeze reconstituted solution; avoid freeze–thaw cycles which can denature the peptide.
  • Discard: Follow the specific product’s validated use-by period. Discard if cloudiness or particulates appear.

Mazdutide Dose and 10 mg Vial Math

A 10 mg vial mixed with 3.0 mL contains about 3.33 mg of mazdutide per mL. On a U-100 syringe, 1 unit marks 0.01 mL, so each unit represents about 0.0333 mg, or 33.3 mcg. The primary mazdutide dosage table above keeps peptide mass, syringe units, milliliters, frequency, and duration together so each row can be interpreted without a separate calculator.

  • Concentration: 10 mg ÷ 3.0 mL = approximately 3.33 mg/mL.
  • U-100 conversion: 3.33 mg/mL × 0.01 mL per unit = approximately 0.0333 mg per unit.
  • Volume check: milligrams ÷ 3.33 mg/mL = milliliters; milliliters × 100 = U-100 units.
  • Vial limit: calculations cannot exceed the 10 mg total mass contained in one vial.

Published clinical trials used manufactured study products under protocol-controlled conditions. Vial arithmetic can translate concentration and volume, but it cannot establish bioequivalence, sterility, stability, or an appropriate dose for any individual.[14]

Mazdutide Dosing in Phase 2 and Phase 3 Clinical Research

Mazdutide dosing has differed across phase 1b, phase 2, and phase 3 research. Early randomized studies evaluated multiple once-weekly dose levels and escalation designs in adults with overweight or obesity and in patients with type 2 diabetes.[1][12][13] A phase 2 trial in Chinese adults with overweight or obesity studied 3 mg, 4.5 mg, and 6 mg dose groups over 24 weeks.[2] A separate phase 2 diabetes trial evaluated the efficacy and safety of mazdutide in Chinese patients with type 2 diabetes.[4]

Later work expanded the evidence base. GLORY-1 studied 4 mg and 6 mg in Chinese adults with overweight or obesity for 48 weeks.[3] A United States phase 2 clinical trial evaluated groups assigned to 3–6 mg, 10 mg, or 16 mg escalation targets in a diverse population.[6][14] Separate research evaluated a 9 mg target in phase 2 and GLORY-2 phase 3 programs.[7][8]

A phase 1b study of mazdutide and later controlled trials assigned participants to different dose groups. Some research included 6 mg mazdutide, 9 mg and 10 mg targets, and a 16 mg mazdutide target under protocol-controlled escalation. These are study-specific doses of mazdutide, not a universal mazdutide dose. Trial population, formulation, escalation rules, monitoring, duration, and jurisdiction all matter. The locked chart above remains a calculation display for this 10 mg vial and has not been changed by the clinical evidence review.

Mazdutide for Obesity and Type 2 Diabetes: Dual Receptor Action

Mazdutide is a dual GLP-1 and glucagon receptor agonist. GLP-1 receptor agonist activity is associated with glucose-dependent insulin signaling, delayed gastric emptying, and appetite-related effects. Glucagon receptor activity can influence hepatic energy metabolism and energy expenditure. Researchers are studying whether combined receptor activity produces a different metabolic profile from single-receptor medicines.[5][11]

Mechanism does not predict an individual response. A laboratory calculation also does not show whether the reconstituted material matches the composition, exposure, or quality controls of the product used in a clinical trial. For that reason, clinical outcomes and safety observations must remain tied to the studied population and formulation.

Body Weight Change and Waist Circumference Outcomes

Randomized trials reported body weight change from baseline, body weight and waist circumference, glycemic markers, lipids, and other cardiometabolic outcomes. Participants receiving mazdutide were compared with placebo or active-control groups according to each protocol. The phase 2 obesity study found greater mean body weight reduction with mazdutide than placebo at the studied endpoints, while the phase 2 diabetes trial reported changes in HbA1c and body weight in patients with type 2 diabetes.[2][4]

In GLORY-1, adults with overweight or obesity assigned to mazdutide had greater reductions in body weight from baseline than those assigned to placebo at 48 weeks. The proportion of participants achieving weight-loss thresholds was also reported, but achieving weight loss in a trial does not predict an individual outcome.[3] The United States phase 2 study also reported body weight loss across evaluated dose groups, but the size and precision of an effect depend on the trial design and population.[6]

Across different mazdutide doses, published research reported dose-dependent body weight reduction in some analyses. That does not define an optimal mazdutide dose, because tolerability, trial design, and population differed. Systematic reviews and meta-analyses combine several trials and can strengthen estimates of average efficacy and safety, but they also inherit differences among the included studies. Recent analyses found body weight reduction and metabolic changes across randomized controlled trials while noting heterogeneity and the need for longer follow-up.[9][10] These findings describe groups, not guaranteed results.

Efficacy and Safety of Mazdutide: Evidence Limits

Across clinical research, gastrointestinal events such as nausea, diarrhea, vomiting, reduced appetite, and abdominal symptoms were among the most commonly reported adverse events. Frequency and severity varied by dose, escalation design, population, and study duration. Higher doses of mazdutide were evaluated only within controlled protocols, and their safety profiles cannot be inferred from vial strength alone.[1][2][3]

Dual agonist activity makes cardiovascular, gallbladder, pancreatic, hepatic, renal, and glycemic monitoring relevant in controlled research. Trial eligibility rules excluded some higher-risk groups, so the evidence does not automatically extend to every population. Published averages also do not rule out rare or delayed events. Meta-analyses support an overall summary of observed safety but cannot replace product-specific quality testing or medical oversight.[5][9][10]

This page is educational and presents research calculations. It does not recommend treatment with mazdutide, diagnose a condition, or establish that any research product is suitable for human use.

Regulatory Status and Study Context

The development of mazdutide includes obesity or overweight and type 2 diabetes programs. Regulatory status varies by jurisdiction. Innovent reported that China’s National Medical Products Administration approved mazdutide for adult long-term weight control and for glycemic control in adults with type 2 diabetes.[15] That authorization does not establish approval in the United States or every other country, and it does not validate independently supplied research material.

The names IBI362 and LY3305677 appear in trial records and publications. ClinicalTrials.gov identifies the United States phase 2 study as NCT06124807 and provides its protocol-level eligibility, intervention, and outcome details.[14] Readers should check the current regulator and trial registry for the jurisdiction and study in question.

Storage, Handling, and Product Provenance

The four reconstitution checks and temperature language in the primary chart are preserved exactly. A concentration calculation does not establish chemical stability or a universal discard period. Follow the exact label and validated lot-specific instructions for the product being handled.

The linked Pure Lab Peptides page identifies the pictured 10 mg research vial and supports supplier and product provenance only.[16] It is not used as scientific evidence for clinical efficacy, safety, or trial dosing.

Mazdutide Dosage FAQs

What concentration does a 10 mg vial make with 3.0 mL?

It makes approximately 3.33 mg/mL. On a U-100 syringe, 1 unit equals 0.01 mL and therefore represents about 33.3 mcg at this concentration.

Is a U-100 syringe unit a peptide unit?

No. U-100 units are volume markings. The peptide amount depends on the vial mass and final reconstitution volume.

What mazdutide dose did clinical trials study?

Clinical research evaluated multiple once-weekly dose levels and escalation targets, including phase 2 and phase 3 programs. A trial dose is specific to its protocol and should not be converted into a universal recommendation.[6][7][8]

Is mazdutide the same as a single GLP-1 receptor agonist?

No. Mazdutide is designed as a dual GLP-1 and glucagon receptor agonist. That mechanism and its evidence base should not be treated as interchangeable with a single-receptor compound.[11]

Do body weight results predict an individual outcome?

No. Trial results summarize groups under controlled conditions. Individual outcomes can differ, and research-vial calculations do not establish equivalence to clinical-trial products.

Where can the research record be checked?

Peer-reviewed publications and ClinicalTrials.gov provide study designs, populations, endpoints, and safety findings. The references below link directly to those records.
